Your health insurance policy is a contract between you and your insurance company. We are not a party to that contract.

This list may not be all-inclusive or plans may have had participation changes. Coverage and participation status can change without prior notice. We will confirm with you at the time of scheduling an appointment whether or not we participate with your plan.

If you will be scheduled for a procedure that involves another facility or physician/provider, those entities will verify in advance that they accept your insurance. You should, also, verify that facility or physician/provider accepts your plan by calling them or your insurance carrier. These may include other physician specialists, laboratories, hospital or surgical center, anesthesiologist, etc., over which we have no control.
